Appropriate activity
Walking, strength and everyday tasks can be introduced within the limits and monitoring requirements set by your treating team.
Heart health
After a heart problem, everyday effort can feel uncertain. This pathway focuses on appropriate activity, strength and confidence, with the treating cardiology team setting the clinical direction and the level of supervision you need.

The starting point
Adults with stable heart disease or a clinician-identified cardiovascular risk who need ongoing functional support. Suitability after a heart attack, procedure or surgery depends on the treating team’s advice and available supervision.

Your programme is shaped around your health, preferences and clinical needs.
Walking, strength and everyday tasks can be introduced within the limits and monitoring requirements set by your treating team.
Work through concerns about movement, recognise your agreed limits and discuss symptoms that need medical review.
Nutrition, sleep and practical behaviour support can help you follow the wider advice provided by your cardiology team.
Your journey
Review the cardiac history, discharge advice, current symptoms, medicines and exercise clearance.
Confirm the right setting, supervision and goals with the treating cardiology team.
Practise suitable activity and daily routines within the agreed precautions.
Review function and symptoms, and return new concerns to your clinician before progressing.
The right measures help the team understand whether the plan is useful and what should happen next.
How measurement worksMeasures are chosen by the clinician. These are examples, not a standard test package.

Working with your hospital
Hospital care and medically supervised cardiac rehabilitation have specific roles after a cardiac event. MiMo360 support must fit the discharge plan and cannot substitute for monitoring that your risk requires. Ongoing community support is considered once your treating team confirms the appropriate setting and level of activity.
Our referral approachYour treating team must decide when, where and how rehabilitation begins. There is no single waiting period or exercise plan that is suitable for everyone.
No. If you need medically supervised cardiac rehabilitation, that remains essential. Continuing support should complement the programme and precautions your cardiology team recommends.
Stop the activity and seek urgent medical help. New chest pain, collapse or severe breathlessness should not be managed through a routine website enquiry or exercise appointment.
This is not emergency cardiac care. Higher-risk activity requires an appropriately monitored clinical setting. Programme suitability, supervision and professional availability must be confirmed before starting; medical decisions remain with your cardiologist.
